Nurse Dawn Wellness Memberships provide weekly, local, in-home nursing visits for adults and seniors across Washington, Holmes, and Jackson Counties. These plans help you stay ahead of preventable issues, maintain independence, and enjoy peace of mind with consistent nurse support.

For clients who need routine support and gentle oversight.
Includes:
Weekly in-home nurse wellness checks (1 hour; 90 minutes for two clients)
Vitals, oxygen, and heart/lung assessments
Cognitive assessments
Medication review, reconciliation & set-up
Home safety and fall risk checks
Hydration, nutrition, and lifestyle coaching
Companionship
Family & caregiver updates
For clients who need consistent monitoring and ongoing support.
Includes everything in the Essential Wellness Plan, plus:
One add-on service per month
Monthly progress summary and plan review
Priority response for scheduling and communication
Deeper guidance for managing chronic conditions or recovery
For clients who want comprehensive prevention and priority scheduling.
Includes everything in the Preferred Plan, plus:
Two add-on services per month
Early appointment access for schedule flexibility
Direct coordination with your healthcare provider(s) (as needed)
Expanded time for education, care planning, and caregiver support
For clients who need extra hands-on care or condition-specific support.
Optional add-ons available for wound care, foot care, post-op support, advocacy, medication pickup, illness recovery.
Membership Policy: Memberships can be canceled or paused anytime with at least 14 days’ notice through your confirmation email link or by contacting Nurse Dawn directly. Cancellations take effect at the end of the current billing cycle.
Add-on services or special visits outside of your selected membership plan are billed separately and are not included in the monthly subscription cost. Travel fees may apply outside the standard service radius.
