A personalized visit designed to check in on overall health, safety and daily wellbeing. Includes BP, temperature, heart rate, oxygen level, medication review, home environment and safety check, brief cognitive observation and wellness education.
This visit may also include care coordination with family members or caregivers to ensure everyone involved understands the care plan and next steps.
Ideal for seniors, caregivers, or post-operative clients who need reassurance, accountability, and professional support at home.
Care Coordination & Advocacy Visit – 60 minutes

Assistance with navigating healthcare or insurance paperwork, referrals, scheduling appointments, provider communication or community resources. Review, scheduling and organization of medical tests and upcoming appointments. Updates and communication with family members or caregivers.
(Primarily discussion-based, minimal physical care. Think of this service like having a nurse in the family — someone who keeps an eye on things, answers questions, and makes sure nothing slips through the cracks. I can visit as often or as little as you need, whether at your home, a care facility, post-acute rehab center, or hospital.)
Medication Setup & Reconciliation – 30-45 minutes

Review and organization of medications and supplements, pill-box setup, and adherence education.
(Non-clinical support service focused on organization and education. This service does not include medication administration or changes to prescribed treatment plans. Service can be provided weekly, bi-weekly or monthly. )

Injection support and education for clients who self-administer prescribed injectable medications. Services include review of injection technique, site selection education, preparation guidance, post-injection observation for comfort and safety, vital sign monitoring, visual medication verification using client-provided supplies and prescriptions .
This service is educational and supportive in nature and does not include medical treatment.
Foot & Nail Care

New Foot Care Visit – 45–60 minutes

Comprehensive nail and skin care with visual observation, trimming, shaping, light callus smoothing, moisturizing, and education on foot health and prevention. (Recommended for first-time clients or those not seen in 6+ months.)
Follow-Up Foot Care Visit – 30–45 minutes

Maintenance care for returning clients. Includes nail trimming, light smoothing, and moisturizing. (Focused on upkeep and continued mobility.)
Group Foot Care (Follow-Up Visits)
$55 per client | Minimum of 4 clients per group
Perfect for senior centers, assisted living communities, or group homes, this option provides affordable, professional foot care in a convenient group setting. Each client receives the same thorough, one-on-one care provided during individual visits, including nail trimming, filing, light callus smoothing (as appropriate), skin observation, and general foot care education and recommendations.
Details:
Minimum of 4 clients scheduled at the same location and time.
$55 per client for established clients only.
Services must be completed during a single group visit.
Discount cannot be shared or transferred outside of the scheduled group.
New clients attending a group session will receive a 20% discount on their initial visit.
All clients must have an updated consent form and health history on file before service.

Foot care services are non-invasive and focused on comfort, hygiene, prevention, and education. Services do not include medical diagnosis, wound debridement, treatment of active infections, or surgical procedures. Clients with concerning findings are referred to a podiatrist or medical provider.

Non-invasive assessment and monitoring of surgical incisions or existing drains to support post-operative recovery. This service includes visual inspection, observation of healing progress, assessment of surrounding skin integrity, drain placement observation, and patient education on signs and symptoms that require medical follow-up.
Nurse Dawn Concierge Services does not provide wound treatment or any invasive care requiring a physician order.
Services are limited to non-invasive, visual observation only. No wound measurements, probing, cleansing, dressing changes, irrigation, drain management, or device manipulation are performed.
Open, draining, infected, or complex wounds are referred to the appropriate medical provider.
Nurse Dawn provides observation-based education and verbal guidance to support client or caregiver self-care of existing drainage systems, as directed by the client’s medical provider.

All post-operative services are provided in accordance with the client’s surgical discharge instructions and are limited to non-invasive observation, monitoring, education, comfort support, and care coordination.
No medical treatment, wound care, dressing changes, drain management, or invasive procedures are performed. All supplies and prescribed equipment must be provided by the client.

All post-op recovery support services must be scheduled in advance. Clients must complete all intake and consent forms before the visit to ensure your entire appointment time is spent on care—not paperwork.
Cancellation Policy:
Cancellations must be made at least 24 hours in advance to receive a full refund. Late cancellations may incur a fee.
