A personalized visit designed to check in on overall health, safety and daily wellbeing. Includes BP, temperature, heart rate, oxygen level, medication review, home environment and safety check, mini-mental status exam and wellness education.
This visit may also include care coordination with family members or caregivers to ensure everyone involved understands the care plan and next steps.
Ideal for seniors, caregivers, or post-op clients who need reassurance, accountability, and professional support at home.
Care Coordination & Advocacy Visit – 60 minutes

Assistance with navigating healthcare or insurance paperwork, referrals, scheduling appointments, provider communication or community resources. Review, scheduling and organization of medical tests and upcoming appointments. Updates and communication with family members or caregivers.
(Primarily discussion-based, minimal physical care.)
Medication Setup & Reconciliation – 30-45 minutes

Review and organization of medications and supplements, pill-box setup, and adherence education.
(Non-clinical support service focused on organization and education.)
Think of this service like having a nurse in the family — someone who keeps an eye on things, answers questions, and makes sure nothing slips through the cracks. I can visit as often or as little as you need, whether at your home, a care facility, post-acute rehab center, or hospital.

Comprehensive nail and skin care with full assessment, trimming, shaping, light callus smoothing, moisturizing, and education on foot health and prevention.
(Recommended for first-time clients or those not seen in 6+ months.)

Maintenance care for returning clients. Includes nail trimming, light smoothing, and moisturizing.
(Focused on upkeep and continued mobility.)

Perfect for senior centers, assisted living communities, or group homes, this option provides affordable, professional foot care in a convenient group setting. Each client receives the same thorough, one-on-one care provided during individual visits, including nail trimming, filing, callus reduction (as appropriate), skin and circulation assessment, and personalized recommendations.
Details:
Minimum of 4 clients scheduled at the same location and time.
$65 per client for established clients only.
Services must be completed during a single group visit.
Discount cannot be shared or transferred outside of the scheduled group.
New clients attending a group session will receive a 20% discount on their initial visit.
All clients must have an updated consent form and health history on file before service.
Ideal for organizations or facilities wanting to keep residents healthy, comfortable, and on their feet — all in one visit.

Professional care for surgical or chronic wounds, including cleansing, dressing changes, and drain management per MD orders with client-provided supplies.
Assessment, documentation, and dressing change provided under current MD orders using client-provided wound care supplies. Includes wound-care log entry and education on wound progression.
Additional time billed at $20 per 15 minutes for multiple wounds or complex cases.
Medication Assistance / Organization – 30-45 minutes

Help with medication timing & education, safe storage, and verifying prescriptions match discharge orders.
Compression Garment – 45-60 minutes

Assistance with applying, removing, or washing post-surgical compression garments as ordered.
Vital Sign & Safety Check – 45 minutes

Quick visit to monitor blood pressure, pulse, temperature, wound drainage, comfort level and mental status— ideal for same-day discharges or peace-of-mind checks.

This option is perfect for short-term check-ins, flexible recovery support, or families who want professional oversight without committing to a full care block.
All post-op services require current physician orders for wound, drain, or catheter care. Supplies are provided by the client. Travel beyond 35 miles of 32428 may incur an additional fee.
